The following important information is provided for you as you prepare for your initial visit.
What You Can Expect at Your First Visit
For adult clients, the first appointment is for assessing and understanding the problem(s) for which you are seeking assistance. For minors, the evaluation process is a two-step process with initial separate appointments with the parent(s)/guardian(s) and the other with the child or teenager. The evaluation phase is important to planning appropriate care and treatment. After the evaluation, should it be apparent that a higher level of care or treatment is needed, other referral resources will be provided.

What paperwork do I need to bring to my first appointment?
Please download, print and complete the New Client Forms to bring with you on your first visit.
For children and teenagers under the age of 18, please complete the New Client Paperwork for Minors.
For Adults, ages 18 and over, please complete the New Client Paperwork for Adults.
